To order repeat medication, tick the items you require and bring or post the slip to the health centre. If you don't have a slip you can write to us.

Collect your medication from your chosen pharmacist 48 hours later (allow extra time at weekends).

New patients will be asked to complete a form indicating which pharmacist they wish to use.
