Make an appointment
Our telephone lines are open at 8am each morning. If you require an emergency on the day appointment we would suggest you ring as early as possible, as these appointments can be booked up quick. You can also book an appointment through the patient access area of this site. Appointments available online have recently increased to include emergency as well as routine appointments with a GP.
Order Repeat prescriptions
Repeat prescriptions are issued at a doctor’s discretion, normally for people on long-term treatment. Printed slips are provided for regular repeat medication. To order repeat medication, tick the items you require and bring or post the slip to the health centre. If you don’t have a slip you can write to us.
Your FP10 Repeat Prescription form will be available for you, or at your chosen pharmacist, within 48 hours and medication can be collected thereafter.
New patients will be asked to complete a form indicating which pharmacist they wish to use.
We regret that our staff cannot accept requests for repeat prescriptions over the telephone. However, you can now order your repeat prescription from the Patient Access section of this website.
Obtain Test Results
You can obtain your test results from our receptionists. Allow a minimum of three working days to elapse and then phone (0117 301 2080) or call reception between 2.30pm and 6pm.
Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the practice. It is your responsibility to check your results and to make an appointment to discuss them with your doctor if your are advised to do so.
Request a Sick Note
Please remember that all employees can self-certify for the first seven days of illness. A self-certificate is available online at: http://www.hmrc.gov.uk/forms/sc2.pdf or paper copies are available from reception. Should you require a sick note to cover a period greater than seven days, please speak to Reception who will advise you on the best option to obtain your note.
Our premises are fully accessible to the disabled. The practice has a hearing loop installed for patients who require this facility. Please ask at reception for this to be switched on for you. If you require any additional assistance, please ask at reception.
